All of the following are required for your application to a PGY1 or PGY2 residency to be complete:

• Official college transcripts (pharmacy and pre-pharmacy coursework) - these should be in a sealed official envelope
• Complete the online Shands HealthCare application
• Three (3) letters of reference (using the downloadable forms below; most applicants' references send a letter and this completed form)
• Letter of intent (usually one page in length)
• Curriculum vitae
